General comments
Onderhoudsdosering vancomycine afhankelijk van gemeten spiegels en van nierfunctie, zie bij vancomycine en bij TDM vancomycine
E. faecalis is meestal amoxicilline-gevoelig, E. faecium is gewoonlijk amoxicilline-ongevoelig.
Eventueel gentamicine 1 dd iv toevoegen gedurende 3 dagen.
