Bij aangetoonde kolonisatie van de luchtwegen met Pseudomonas spp.

benzylpenicilline iv 12million. IU continuous per 24 hours
Remarks:

Bij recent verblijf in land met hoge prevalentie penicilline-resistente S. pneumoniae.

levofloxacine po 500mg 1dd

General comments

  • Altijd bloed- en sputumkweek insturen.
  • Tijdens influenza-seizoen flucloxacilline ipv penicilline
  • * Risicofactoren Legionella:
  1. Recent bezoek aan het buitenland
  2. tijdens een Legionella epidemie
  3. Onvoldoende verbetering na 48 uur behandeling met een beta-lactam antibioticum
  • * Legionella antigeentest negatief: penicilline iv door

        Legionella antigeentest / PCR positief: switch naar levofloxacine po

  • Bij onvoldoende opknappen of achteruitgang, sputum/keelwat afnemen voor PCR op atypische bacteriele verwekkers. PCR op sputum is gevoeliger dan op een keelwat. PCR bacteriele atypische verwekkers omvat: Legionella pneumophila, Mycoplasma pneumoniae en Chlamydia pneumoniae
